is voluntarily participating in the Lake Travis Music Theatre, Inc’s Rising Stars Company. I understand that any class/production for which my child participates may require physical exertion at times which could cause physical injury and am fully aware of the risk hazards involved. I represent and warrant that my child has no medical condition that would prevent his/her participation in the program. I agree to assume full responsibility for any risks, injuries, or damages that he/she might incur as a result of participation in this program. I also agree to the use of my child's photo or video on LTMT's website or social media solely for purposes of class/performance promotion. I knowingly, voluntarily, and expressly waive any claim against Lake Travis Music Theatre, Inc. or its contracted venues for any injury or damages my child may sustain as a result of participating in this program. I have read the above waiver and release of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.
